This statement applies to the privacy of users of this Website including the Online Outpatient Appointment Booking and Online Enquiry Form. All personal data collected from you through such channels will only be used for the specific purposes mentioned at the time of collection or for purposes directly related to facilitate you in making an appointment or enquiries.

Your personal data will be kept confidential and will not be disclosed to other person without your consent. Your personal data may also be disclosed in case STH is required to do so mandated by law.

1. Information Collection
1.1 To make an appointment, STH may collect your full name (Chinese and/or English), date of birth, phone number, address and email address (where applicable), which are specifically and voluntarily provided by you to proceed to submit the appointment. STH will store and maintain your personal information so provided. If the appointment is made on behalf of another person, the person making the appointment should have obtained your proper consent before providing his/her personal information for such purpose.
1.2 Upon arrival at STH, we may collect your personal information prior to the Services to complete your registration process, including your full name (Chinese and/or English), identity card number, travel document number, date of birth, phone number, address, email address (where applicable) and information of your next-of-kin. Such personal information will be checked against the information provided for the appointment via the Online System. STH will also store and maintain your personal information as patient record.
1.3 Being your healthcare provider, STH may ask you to provide your personal data (including health information) or obtain from any appropriate third party regarding your medical history and any relevant information for your health care purposes and/or generally for medical care and treatment purposes and/or for the purposes stated below.

2. Information Use
2.1 STH uses your personal information for the purposes stated at the time of collection, such as providing medical care, managing patient records and payments, improving our services, communicating with you about your treatment, meeting legal or regulatory requirements, and ensuring the safety and security of our operations. We will not use your information for other purposes without your consent or unless required by law.
2.2 STH will also use your personal information and history of receiving Services to send out SMS reminders for the upcoming appointments.
2.3 Your personal information may be used by STH for anonymized continuous quality improvement purposes.

3. Disclosure to Third Parties
3.1 STH may provide your personal information to other entities within STH for planning and providing Services to you.
3.2 By accessing the third-party electronic platforms during the booking process, the personal information that is provided shall be transferred to such third-party service providers for the purpose of completing the appointment, system maintenance, or directly related purposes.
3.3 For providing healthcare services to you and the uses stated above, STH may transfer your personal data to other doctors/healthcare providers/third-party service providers who require it for purposes for providing and supporting healthcare services.
3.4 Personal information may be disclosed to law enforcement, regulatory, or other government agencies, or to other third parties, in each case to comply with legal or regulatory obligations or requests.

4.1 STH will retain the personal information provided for a reasonable period of time to implement the purposes for which such personal information is collected or to comply with the applicable laws, regulations and regulatory requirements or rules of Hong Kong. The personal information would include but not limited to your name, email address, phone number, date and year of birth etc.

5. Data Access and Correction
5.1 You have the right to access and correct your personal information held by us. To exercise these rights, please send a written request to STH by email to sth@sth.org.hk or post. We may charge a reasonable fee for processing access requests. We will respond to your request within a reasonable time in accordance with PCPO requirements.

6. Accuracy and Security
6.1 STH will ensure appropriate procedures in place to maintain, so far as is reasonably practicable, the accuracy, completeness and relevance of the personal data used by STH in relation to the purposes for which the personal data are collected.
6.2 STH will implement appropriate measures to protect the personal data we hold against unauthorized or accidental access, processing, erasure, loss or use.

7.1 Any change or update to this Privacy Policy Statement will be effective immediately upon posting on this website.

8. Governing Versions
8.1 In case there is any inconsistency between the English version and the Chinese version, the English version shall prevail.
